Each church is required to bring at least two officials per team registered. If you are able to bring more, please list their information in the 2 extra spaces below. Each church is encouraged to provide at least 1 QuizMaster and 1 ScoreKeeper for every 2 teams registered.

Please give an estimate of the number of lunches to be purchased by those from your church. (If you have already indicated your lunch count when registering another team, please enter a zero on this registration.) Lunches will be purchased at check-in on Friday evening (do NOT send payment with your registration fee), but the estimates will help in planning.

A fully-staffed nursery will be provided and funded by the Kentucky District JBQ Ministry.
